You can get suprisingly close to it without getting killed. The collision detection is just for your small mouth and not the whole body so only the mouth has to avoid it.
 
Its more fun (in my opition) with the sound off. I remember the first time i plaid it was late late at night and the sound of the rat dieing scared the crap out of me 😀
