The form is VERY buggy. I had to enter my information in both Shipping and Billing sections even though I had the box checked to use the same address for both.

humm, after lying to the form with ny and nyc zip code, I got a confirmation, I sent the print out to customer service and hope they process it correctly.

Cool, good luck. My brother wants one but I have another possibility from a buddy so I decided to go that route instead of through onSale.

30 minutes clear to get to dentist's office. Figure 40-45 with traffic, but I shouldn't have any paperwork to deal with.

They stop taking my insurance company as of... I think February. Less than six months away, so I can't make another appt., and will have to go elsewhere. :(

It occurred to me... there's all sorts of things about creating non-drowsy formula medicine.

Just add a little bit of caffeine to the drug. All set!

They were supposed to verify all the information that you entered matched the Amazon order, I thought. At any rate, the form was extremely buggy and crappy.
