Are you supposed to capitailze each word in "To whom it may concern"?

And is it followed by a colon or comma?

Word seems to default it was all capitals followed by a colon..I've always lower cased each word except for To and put a comma after..

If you're using "To Whom It May Concern" in a friendly letter, you've got bigger problems than capitalization!
