Software voltage monitoring can be very inaccurate. You really need to put the system under a heavy load and then use a volt meter to check the voltage at a free molex connector. Anything withing +/-5% is good to go. But that doesn't mean the PSU is good. As a PSU ages it is no longer able to supply the same amperage it once could when it was new. The average life of a PSU runs about 5 years. So it wouldn't be a surprise to find that the PSU is under powering the system. You really won't know until you get a new PSUY and try it in your system. At this point it would be a good idea to get a new PSU anyway.

Now at the same time the video card itself could be at fault too. When you get artifacts like that it could be several issues. The card could be Overheating, under powered and/or the VRAM may be going bad.

So i think you need to start with a new PSU either borrowed or bought new. If the video card still exhibits the same problem then you have a bad card.
