MN PC Tech does this kind of thing I believe. They have some Youtube videos of some of their high-profile case customizations that are pretty impressive.

That said, this is generally not going to be cheap.

DARWINmachine is making some very unique cases these days.

But for a custom wooden case, your best bet would be to lurk in the work/project log sections of the popular forums and approach a builder there.
