For any of the experienced escrow users, I was wondering where can i get my authorization form? the one you fax to escrow with a license and cc image. anyone?

If this is an eBay purchase, be careful of the many people who use fake escrow sites to collect your money then run.

 

Escrow, the only escrow I deal with is property taxes.
