MS Lync - Some are First Name Last Name, others are Last Name, First Name. Don't ask me why.
 
Skype, all day everyday. And we have no convention on user names. It's whatever the person chose when they created the account.
 
Our VOIP interface has an IM feature...although working with my SO as my boss...we have to remind ourselves sometimes that all of those messages are stored and can be easily read by the couple people above her.

I know you're a developer - do you interact with people from India? When I had an offshore team all they used was Skype but none of our US team really did.
 
It was Meca (2004) which ran on AIM (!), then Inbit Messenger now 24im enterprise. ID is three characters of title+vessel code.
