I would send you an empty spreadsheet, but I'm not feeling ambitious. What exactly are you looking for it to do? Just provide you with a bunch of categories to budget in?

These really vary by person.

My budget spreadsheet has a column for alcohol. My friends has a column for school supplies. Make your own, it takes 15 minutes, and you might even learn something new about Excel.

I have one, but it is so closely tied to my finances, I can't clean it up for general use.
