would you do an back arm, and just adjust the lever to your desire?

We have a tilt on it. Without it the angle would be totally unacceptable.

However, I got one of those arms once for a different tv, and the TV ends up being about 6 or 8 inches off the wall with no way to push it any closer even when the arm is fully collapsed. It looks like crap with the exposed arm behind it.

I am sure they make better arms that are not as obtrusive, but I was shopping at Monoprice and well you get what you pay for :)

We moved into our current house over 4 months ago and above the gas fireplace was the only logical place to put it.

We've watched 2+ hour movies down there and have not had any neck strain issues. The couch is 10' away. The fireplace has a fan that blows the hot air out and the temperature directly above the fireplace doesn't get hot when it's on.

Mine's right in front of my fireplace. I know that doesn't help. :p

My cousin's is over his fire place, and tilted down it looks pretty nice.

I would expect the heat to reduce the life of the display.

If there's little or no rise in temperature in the area in which the TV is mounted, it should be safe. (The user manual for the TV should list a maximum ambient operating temperature.)

I would be more concerned that there would be insufficent stud above the fireplace to properly mount a flatscreen.

Mine is over the fireplace. No neck strain. This is a gas fireplace. It is really the only good spot to put it and have all seats facing the TV.

Ditto (except we do not use our fireplace due to chimney damage).

It is a retarded idea. How many people stand in front of a painting over the mantle and stare at it for 2 hours straight? Tv's do not belong over the fireplace. Just imagine taking your current pc monitor and raising 4-5 feet above where it is now. It is just bonkers, and an ergonomic nightmare.

Is your monitor 14 feet away?

Yeah, having a fire place is kind of a head scratcher. This house was built in 1979 and they made some *weird* decisions.
