Any advice on how to get insurance to pay for surgery?

I had to have surgery in the summer for a rupture (or something of that nature, forgot what the doctor called it exactly) of one of my muscles. The insurance company oddly said that it was an elective surgery and didn't cover it. I'm now filing a request for them to cover it. Is there any tips anyone has in order to get it covered? I'm thinking of going to my doctor and having him mention it was FAR from elective, the muscle tear caused me extensive pain and prevented me from working out for a while.

I'm also thinking of attaching my own letter explaining the effect the issues had on my life, and the pain it caused. Would this be advisable? or would they just disregard it.
 
Definitely go to your doctor. Most doctors are down-to-earth people that know insurance companies and HMOs suck ass, so it's almost certain he'll take your side. Just a matter of whether your doctor is a cool enough guy put some effort in for you.
 
He's willing to write that it should definitely be paid for, he's amazed that my insurance company didn't cover it right off the bat. How insturmental is the doctor in getting the inmsurance company to cover it?
