You should arrange the contacts in accordance with the pinout of your mobo's USB header, of course. If it doesn't work when arranged in accordance with your mobo's pinout (as I'm fairly sure it is right now), then get out the ohmmeter as one or more of the wires is likely FUBAR.

I'm on vacation, if you want pics it'll have to wait until at least the 27th, and only after I find someone with a digital camera.🙁
