She looks like she has some kind of disorder, maybe endocrine system? I would say Hypothyroidism.

She really needs to go to an endocrinologist and get that checked out.

You could be obese as a female but the face should not be puffy like that. If your face looks like that it is definitely some medical condition that needs to be addressed.
