No, not every band, just many of them.

Well, you have to figure it this way... for a band's first album, they've had their entire lives to come up with the material. They put their heart and soul into it, and it represents everything they have up to that point. For a follow-up, they have to make it not only different and fresh, but there simply isn't the same amount of time and heart put into it. It's certainly not the case for ALL bands, but for most it's pretty reliable. In RATM's case, their first album was just so much more RAW.
