Anandtech survey with a bunch of questions

The questions are pretty straight forward. But where do we send the blood sample and the small piece of skin from the back of the neck?

Are we supposed to write our social security number on the back or front of the envelope?

Get rid of African-American on the poll and put black, it's more correct, since not everyone on this forum is American. Unless you mean African-American, and want non-American black people to vote "other".

Of course not. We are supposed to enclose the credit card in the envelope. The pin # goes on a post-it note attached to the credit card. If I can't find any post-its, can I tape a piece of paper with the pin # on it to the credit card?
