Adding new mkd file into Pervasive.sql 2000i Database Engine

how do i establish a new table in a database engine if i just have the mkd file? I tried the "Create Table Wizard" - checkboxed the Advanced Settings" to select the mkd file, but it doesn't load the existing fields properly. I am working in Pervasive.SQL 2000i. Thanks.
