Q6600 with 4GB RAM on Vista 64. EDIT>> I'm reading Vista uses cached memory instead of leaving free memory laying around. So I suppose the 66MB shown here is fine, but I guess what I am wondering now is if 4GB is enough these days or should I go to 8GB for $60 more? I don't multitask THAT often, but I do load games which seem to take a while.

If your system isn't using all the RAM it has now, I wouldn't bother expanding it. If you are going to W7/8/8.1, I would recommend it.
 
Cached memory is memory that's being used to cache the contents of your HDD. Data will be instantly evicted from the cache if an actual application asks for it, so it's not really hurting you to have it sitting there.

Given that, you're actually only using about half of your system memory. If that screenshot represents your normal steady-state workload, then I'd say you're fine with 4GB.
