Inconsistent 3D performance with 8800GT.

Just built a new PC with an EVGA 8800GT card and it's not performing as expected. 3DMark score is ~2500. I have gotten it to run properly on two separate boots with Portal running at ~300fps, but the rest of the time it runs at 20-40fps in the exact same area and with the same settings. Team Fortress 2 performs even worse under any settings.
- Did it work normally at one time, or has the problem always existed?
It did work under these circumstances, but I'm not sure if it was random or not:
1. On installation, thanks to a vague manual, I connected a SATA power adapter that it came with without realizing I could've used the PCI-e connector. Bad performance, so I connected the proper cable, and after that it was running fine.
2. After it was showing the same symptoms again on the next boot, I reseated the card and reconnected power. Worked fine on reboot.
- Is the problem consistent and repeatable, or entirely random, or semi-random?
See above - seems pretty random, unless I have mice in my case pulling cables or hardware loose.
- I already tried these steps:
Updated motherboard drivers
Updated to newest video card drivers in safe mode and using Driver Sweeper
Manually set PCI-e frequency in BIOS (was set to Auto, switched it to 100)
Reseated card and reconnected power cables
Ensured temps were OK and that the card's fan is running
Currently running Orthos stability test as per EVGA tech support suggestion, no problems so far after 3 hours.

- Intel e8400 Core 2 Duo, no overclock
- EVGA 8800GT
- A-DATA 4GB (2 x 2GB) 240-Pin DDR2 800, 1.2v
- Aerocool Zerodba 500w PSU - 20A and 17A on +12v rails
- Other information that might be relevant
I suspect this might just be a busted card, but any other ideas are appreciated.
