I have a mini-itx case that has a 80mm x 80mm x 15mm fan that has failed. I'm having trouble locating a fan that thin. Any ideas where I should look?

Well, I know Zalman makes an 80x80x15mm fan, manufactured for their gigantic passive graphics-card heatsink to give it just a bit better cooling ... found one over at CaseCooler -- Text for $8.95 + s/h

Also happened to find an Evercool 80x80x15mm fan over at Nexfan -- Text for $3.99 + s/h (if you shop here, you'll want to load up on stuff, flat $6.99 shipping no matter the quantity ...

I'm sure you could find them elsewhere for different prices, now that you know the models for them
