6600GT - OS doesn't start when Nvidia drivers are loaded

Having problems with new 6600 GT card i just bought, straight from the support email i sent chaintech:

System does not come up once display drivers are installed. When using generic windows display drivers system works. Low res, low color, no gaming. But the system works. When i install the Geforce drivers and reboot Windows XP splash screen comes up, after that all i get is a black screen, system is hung. Hard reboot brings up the safe mode menu. Only way i can get it back is to use last known good config(which brings it back to the driverless config), or if i go to safe mode and uninstall the drivers.

Drivers tried include the ones bundled with the card, 66.93 from Nvidia's site and 70.90 beta drivers from www.guru3d.com. All have the same problem. I am indeed plugging in the power connector on the card.

I am replacing a Geforce 3 card. None of these problems exist with that card. It comes up with the drivers loaded no problem. Worked before i put in the 6600GT, and is working now, having replaced the 6600GT.

Ok. I am running a Epox 9nda3 board, which is an nforce3 ultra board. I figure chances are i'll get a few replies here(hopefully) before chaintech even reads what i sent them. Any ideas?
 
Upon further inspection of the card in question i noticed the heatsink on it is loose. Pushing lightly on one corner completely lifts up the heatsink and you can see a gap between the gpu and the hsf(with the thermal goop stretching in between). So i'm going to assume i got a defective card. Nice.
