4-port serial card: Only 2 ports are working

We installed a StarTech PCI4S550 into one of our XP machines only for it not to work. After playing around with all the settings and re-assigning COM ports, still nothing. We pulled the machine, re-imaged it to a bare copy of XP, re-installed the card. Now, only ports 1 and 2 (COM4 and COM5 on our machine) work. 3 and 4 (COM6 and COM7) do not. All the settings are identical.

What am I missing here?
