4 PIN connector?!?!?

Does anyone have a diagram of the 4 pin connectors showing the voltages in it?

Exact question, what are the voltages for;

Red? (+12 VDC?)
Black next to red? (0 VDC?)
Black next to yellow? (0 VDC?)
Yellow? (+5 VDC?)

Yellow is 12votls, red is 5votls and the blacks are the ground wires.
