Did you try to disable v-sync completely?
v-sync together with a limiter won't work,well it won't do what you want it to anyways.

Other than that it would help if you would show us cpu and gpu usage or better yet monitor it yourself while playing.
If cpu usage reaches 100% (hard to believe for your i7) then raise quality and resolution so your vga will slow down your cpu,if your vga reaches close to 100% try lowering quality.

Or maybe it yust has a problem with hyperthreading,so if you like try disabling it,or force the game to run only on 4 cores through affinity.

The only time I got stuttering was in the first chapter, the transition from the forest to town would stutter for half a second to load in all the NPCS, other then that area the game ran fine for me. Are you getting stuttering beyond chapter 1?

Unfortunate issue to have.

I've never seen it stutter except when I first tried to run it with bells and whistles enabled on my crappy GPU. Tried again after upgrade, and it ran smooth.

Do you have the game on an SSD? It can stutter when loading a new area, which it does dynamically all the time.

Also, what resolution are you running at? Are you maybe using too much VRAM?

On my 7950 OC it never stutters. Runs very smooth on Highish @1080P.

I played a little bit more, and it's full of QTEs. What were they thinking? Does the QTE density stay constant throughout the game? If it does, then the stutter is a non-issue because I wont be playing the game.

You can turn QTE's off entirely from the game options, the only part with mandatory QTE's is fist fighting which is a side activity.

My biggest problem with the game, and pretty much why I quit playing the first time through years ago.

It isn't everywhere, though: only that Dragon and the first major boss fight in the next chapter, IIRC. It is part of all of the fist fight side missions/gambling if you want to do that...and is very much a joke for those. Constent counter moves that are absolutely impossible to get off. I just mash buttons furiously until something works, lol.

anyway, it has no place in games like this, tbh. They eliminated them for Witcher 3, thank the gods.

I don't think you can turn them off entirely, but there is an option to simplify them which I immediately turned on. (it makes the game a lot better, like you I cant stand QTE's)
