first usa / united miles visa is now giving 17,500 miles when you get the card. that means you only have to charge $500 a month for 15 months to get a free ticket, and they offer other promotions too.
on my card the fee is $60 and that gets you 1 mile per $1 charged.
i am pretty sure you can upgrade to the gold card for $100 a year and that gives you 5000 more bonus miles.
you can also downgrade the card to a $0 annual fee and get 1 mile per $2 charged.
united also offers a 5,000 mile referral bonus. if you feel like asking me for a referral code please do so but i am not posting here to get referrals. i consider the deal hot in its own right.
also fyi - these cards have a high interest rate. if you are the average card user who has $3000 in a balance and $1000 a month in charges you pay about 3 times the value of the ticket just from the higher interest rate than if you purchased the ticket with a better interest rate credit card.
