100 CD labels + kit Free after MIR Frys Today only

I haven't seen this posted yet, and I know how people love free.
Frys has the meritline Pro cd labeling kit free after rebate.
It includes 100 cd labels, 10 jewel case inserts, 200 address labels, labeling sw, and burning sw???? for $12.99 with a $12.99 MIR.

It is in the SoCal wednesday sales paper, and lasts through today.

The MB store didn't have any on the shelves with the other label kits.
I did find them on the top stock shelves in 2 places.
They also only had 1 rebate form and were handing out illegible copies.
You couldn't read the address or any stipulations.
Made them make me a shiny new readable copy of it.

I picked this up the other day, and when I got home I found that the MIR form they gave me was for $6.99. 🙁 So I don't have to go back to the store to complain (like it would do any good at Fry's), does anyone know of a $12.99 on-line form I can print out? Do you have a copy you can scan and e-mail?
 
Stop saying to put B&M in the title.
There is no Frys online.
There is Outpost, different company, different stock, different prices, different specials.
I would put Outpost if it were Outpost.

YOu should put B&M in your title.

Fry's is only B&M otherwise the title would've been
"100 CD labels + kit Free after MIR Outpost.com Today only"

Grrr.
The unreadable one was the correct one.
The one they copied for me expired two weeks ago.
So I too need the rebate form, or am going back to complain tomorrow.
I can't believe I didn't double check it, this is Frys after all.
