I think all the 4 LED's are in series, that is each of which runs on 3 Volts (normal)
The LED which does not blink is probably first in the series but it doesnt make much sense. If your fan controller drops considerable voltage to lower the RPMs, might be the cause of the Voltage cutting down to around 6 Volts, thats less than 2 Volts per LED, and that means they wont work well.
The pulsing can also be due to a capacitor effect but a fan controller is only a potentiometer to add resistance in the way to drop voltage. If 3 of the 4 LED's are pulsing means " WTF man.. lol ". I could give you a solid reason if all 4 were pulsing, but only 1 is not, no idea why.
