Text if you don't know what I'm talking about.

My favorite is Zao's Liberate Te Ex Inferis, which translates to "Save Yourself From Hell". It's based on a line from the movie Event Horizon (which I still have not seen, I really need to).

Anyway, the album is apparently based around this movie in some way. I really enjoy the album a lot, and I just found the CD the other day and it's strange to listen to it again after so long. I think I'll add Event Horizon to my blockbuster queue now.

edit: if you're inclined, state what the concept is behind the album.

I really liked all of Coheed and Cambria's albums. It's like a four-part concept album.

Didn't realize the first two were concept albums. Definitely love them both. Not a Green Day fan though.

Is that for sure a concept album? I knew their 2nd one (Francis the Mute?) was, but I definitely didn't realize DitC was.

The Wall > *
Deltron is pretty good, as are many of the others mentioned, but nothing can beat The Wall.

Opeth - Ghost Reveries

It's about a son killing his own mother and is mentally being tortured for it (Damn).

stevie wonder's 'innervisions'. at first i had my doubts trying to pinpoint it to a specific theme, but the album notes on cduniverse pretty much summed it up for me:

Wonder tackles the socio-cultural landscape of 1970s America, including drugs, urban life, and crooked politicians, in addition to questions of identity, faith, and idealism.

Yeah. It's the story of a guy going into a coma (from drugs I think) and the dreams, journeys, and battles he has in his mind while in the coma. He finally wakes up and decides to die. IIRC.
