I had this very same co-worker and it also drove me crazy but as you said it was a nervous tic and not going to stop without therapy or drugs so you just have to ignore it.
Well it could be that it was much worse before, and this is what she has gotten it down to after therapy or medication.

I worked with a guy that had that problem...but instead of constantly clearing his throat or sniffing, he whistled. Constantly. All day. Never realized he was doing it, he got so used to it. Not a really loud whistle, but enough to know it's there.

Needless to say when our group switched buildings he was put in a cubicle as far away from another person as possible. lol. I always used headphones, so it didn't bother me, but it drove some others nuts.

Dammit...did I work with you? ():)

I've had the problem of "whistling under my breath" for decades...I usually don't realize I'm doing it...until someone points it out to me... :oops:

On a silent count of three, I quietly, but quickly, open the door, being careful to position myself behind it for 3/4 cover as I scan the room for traps. Then I dash past the co worker, swiping my axe as I go by on a route that allows me to dive to safety behind his cube wall with only a quick step onto his desk.

Fuckin lol... good thing I didn't have a mouthful of coffee. Sounds like you've thought this through pretty well eh?

And to answer the OP: I'd probably ignore them. About an 80% chance of ignore and 20% chance of asking "what's wrong?"

I would have put my headphones on, like several others have mentioned.

Having said that, sometimes I caught myself doing something like that (sighing, tsking, etc.) without realizing that I've done it and/or how long I've been doing it. I of course stopped as soon as I realized, but I sort of wished that one of my co-workers would let me know if I did that, so I could stop. In other words, the people who do these things probably don't realize they're doing it.

Usually when I do that it's because I'm reviewing legacy code that was written poorly.

Of course, I have an office, I'm not bothering anyone...
