The bruises are from the seam of your shirt being pressed into your skin when your shoulders are making contact with equipment. You can see a perfect imprint from each side of the seam in your bruises.

Looks like localized bleeding from abrasion. If there is something resting on your shoulder, which it looks like, that's what's doing it.

I can get something similar when wearing a heavy backpack that isn't secured to my waist properly. (so the weight is on the shoulder straps and not the hips)

You're giving yourself hickies (broken capillaries causing blood leakage, very minor surface bruising.) If it bothers you, figure out what is rubbing on your shoulder and do something to change it (padding, some kind of skin lubricant, whatever.)

It's the shoulder pads on the standing calf raise or hack squat. Guaranteed.

Agreed. At my gym at work, we have a leg press machine that did something similar to my shoulder. It's the type where you lay down and brace your shoulders against some padding, and push against a static plate, and slide your body backwards, pushing the weight up. It put a similar looking welt on my left shoulder that lasted for a couple of days.

Yep, and the seams in your shirt make it worse.

Buy a shirt with no seams on the shoulders. Wear that on any day you put pressure on your shoulders... like leg day.
