I have a relative that has a barely driven 2002 BMW 330XI with ONLY ~20k+ miles, that's like 4k miles a year. The car is fully loaded with sports package and winter package i think. My only concern is that this car is barely driven and sometimes it would go for a month+ without being driven, it would just sit in the garage.

The car itself is in perfect condition from the outside to the inside, the only problem I think I have with it is that the car is louder than normal with it starts and takes a bit longer to warm up, like the engine's noise would go back to normal after driving it for a 5-10mins.

Is this easy to fix with a complete tune up / fluid change? or should I stay away? he's probably going to give me a good deal on this car.

No, he is ripping you off. Do not buy it.
It's bound to fall apart very soon given the problems you mentioned.

Since I am local in NYC, I will do you and him a favor and take it off his hands for $15k.

Manual or auto btw?

It's Auto, he'll probably try to get ~16-18k off me. We'll see, there's a guy above you that offered 19k for it.

the loud engine noise from Cold is normal same thing happens on my 3 series. someone once told me what it is. Some sort of secondary heating thingy... to get the car to operating temprature faster

if you sit for a bit it goes away mine lasts no more than a few minutes.
