Placed an auction up and stated will only ship to domestic USA. Winner of auction is from Canada.

Should I sell it to him and charge a crazy shipping and handling fee? (item was far below blue book).

Should I not sell it to him and re-list it? If I do, can I get my fees back?

If you charge him a rediculously high shipping and handling fee, your Feedback will likely be bad. Better to just charge him the difference in shipping and give him a warning to pay more attention to auction guidelines next time.
