Most likely it was written in C or C++. It is still closed source. There is an open source xCom clone on SourceForge that uses the (now open source) Quake 2 engine.

so people who have created fixes for it dont have access to the code either - how do they do it?

You can do some modifications of objects/characters using a hex editor, although I think most of those indices were determined through trial and error.
