Alright, so here's the rundown. Xbox 360 live, works great. For the most part. I can connect, no lag usually, pretty good connectivity all in all. However, my one friend that I actually want to play against I can't connect to for some reason. He also runs his Live just fine, and has no issues playing online. In fact I would say he has LESS issues than me, as Ive had some problems connecting to people. Which leads me to believe the problem is on my end.

I've emailed tech support, all they say is try hard wiring the router if you are behind a router. Well, that would be fine. Except for some reason that doesn't seem to work either.

Fiber optic internet coming from the wall going into a D-link router. Router feeds the 360, and computers throughout the house. I've tried hardwired approach straight from wall to 360, doesn't work AT ALL. Going through the router it works, just can't connect to my buddy. And this is on ANY game. We've tried 5 different games, none of them work. Very very annoying.

Anyone have any tips/suggestions?
 
Try DMZ'n(or just bypass the dlink) your xbox and come back. Also dont expect us to be prompt at 1 am.
 
I recently had the same issues and bypassing the router / opening ports 88 & 3074 / DMZ the xbox seemed to fix the problem. I just went ahead and put the xbox into DMZ.
 
If you are running an older firmware just upgrade it and it opens the ports automatically anyhow. I have mine in the dmz as well though.
 
I also used to have this problem. I could only connect to certain people and others it would drop me. It ended up getting fixed by messing with my router settings, I can't remember exactly what I did, but there should be plenty of help out there on the xbox live forums if you search them.
