"wrinkles on her brain had been flattened"?

I was reading an essay by Joan Didion ("Sentimental Journeys"), and there's an instance of rape in New York, and it was described, "the characteristic surface wrinkles of her brain flattened". what kind of physical brutality must be present to "flatten the surface wrinkles of the brain"? i don't think hitting the head constantly will do this, but i wouldn't know. any ideas?

yeah of course it's a metaphor or an exaggeration, but I want to know where this exaggeration came from, if it was just a beating to the head or if scalp was torn off or what
