I uninstalled, reinstalled, removed all add-ins, installed just Word and nothing else (no excel, ppt, etc.) and I still can't get this thing to work.

A bit of research tentatively identifies ssi1mdu.dll as a Samsung printer driver:

I believe this is correct due to the fact that your Fault Module version: 6.4.34.0, is slightly newer than Samsung driver version 6.4.3.15, listed above.

Which begs the question: Do you have a Samsung printer, or perhaps some other Samsung device that would be involved in printing your Word docs?
