WinXP start menu questions

My StartMenu is a disaster area. I'm trying to get a handle on it. As a first step, I've placed all programs in the All Users.Windows/StartMenu/Programs . But I've got a couple of questions...

1) I noticed that some programs were installed under "All Users.windows/StartMenu" while others were installed under my user account "Mike.Homer/StartMenu". I did not specify the user name for the install so how did the programs choose under which User/Start Menu to appear?.

2) Do you have a particularly well-organized StartMenu? How do you do it? I've also heard about some StartMenu replacements. Any comments?
 
Some programs require an administrator account to install, they usually install for everyone. Other programs default install to the user who installs them. Sometimes the installer asks if you want to install for All Users or just the logged in user. My Start Menu is nice and neat. "All Prgrams" has 7 sub-folders organized by type (Graphics apps, internet, office...) I also don't have 8 million things installed, so that makes things easier to keep neat. I just drag and drop newly installed apps to the proper folder. Nice, neat, and no fuss or muss.

1) When you didn't require a password you made it so you would automatically enter XP without going to the user screen. This means you are automatically logged into the user account. Any software you add from there will be listed as the users.

2) I use small icons, reduce the number of recently used apps/docs, and delete or move to subfolders progs I never access. I think tweak UI has a couple other things you can do to tidy up.
