Windows explorer menu bar displays windows-flag

Every once in a while my windows explorer windows and internet explorer windows go bonkers... they start displaying the windows symbol icon centered right under the title bar instead of the menu bar and I can't get the menu bar back..... it's almost like my explorer has assumed the attributes of a IE pop up window.

Any culprits, sugestions, tools?
