win98 SE startup question

Where are all the places in win98 SE that an entry can be made to make a program run automatically when windows starts up? I once downloaded a very annoying program that simply would not go away. I deleted its entry in startup, unchecked its box in the startup tab of msconfig, and delted its registry entry under HKEY_LOCAL_MACHINE\Software\Microsoft\Windows\CurrentVersion\Run (and any of the other run folders it showed up in), yet the program was still starting up with windows every time. Where else can an entry be made to make a program start up with windows?

HKEY_LOCAL_MACHINE\Software\Microsoft\Windows\CurrentVersion\RunServices

HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\run

MSCONFIG.EXE does cover all of these Registry Keys though. The only other possible place is win.ini, under the [windows] section, the Load= and Run= Entries.
 
I was reading an article posted about a DoS attack on grc.com, and the author mentioned how the zombie programs manange to keep running on a system. He mentioned they put an entry into "the Windows Shell "command/open" key". (quoted from article) What is this?
