Why is my micro SD card saying it's write protected?

There's no write protect slider on it. The Micro SD to standard SD adapter I'm using has a writer protect slider on it, but I've tried it in both positions and it still says it's locked.

This setup has worked in the past..

any ideas? It won't even let me format it.

Which forum is better? I thought about it... digital cameras? It's for my phone, technically.
