Milkdrop is still the bomb visualization. For what? 4-5 years now? Nothing else really comes close. I can't believe people here don't know what it is. OMG teh intarweb!

Get used to dissapointment.

As far as why there hasn't been a better one since 2001, there really is no answer. It is a great idea, and hopefully since it just went open source in May 2005, hopefully someone will improve upon it.

I still occasionally sit and watch the thing to my favorite music. Play it along with New Age stuff and nature sounds and it can be almost hypnotic.
