Why does my TI4200 crash?

Help please.

I recently bought a PNY TI4200 128MB 8x AGP and I've been having major problems with it. If I try to run 3dMark2001, it crashes within the first couple of tests. It has never finished a whole test. Sometimes it will crash back to the desktop and other times it will cause a BSOD. Whenever I try to play games like Simcity 4, it gives me the same issues.

Things I have done (in no particular order, with no success):

Changed power supply from 300w Enlight to 400w ANTEC
Lowered Memory Timings to Standard or Less Aggressive Settings
Removed all other PCI cards
Re-installed drivers (multiple times, multiple versions -- each time uninstalling the last version)
Disabled AGP fast write, 4x mode, etc
Installed new 4in1 drivers (hyperion)
Changed CPU FSB / multiplier / voltage settings

** NONE OF THE ABOVE HAS WORKED! **

Please help me, I have no idea what to do next. Is it my card or am I missing something?

My hardware:
Iwill KK266
Athlon 1.4 ghz (not overclocked)
2 (256MB -- PC133 SDRAM Micron/Crucial)
400w Antec PS
More than adequate cooling, I have checked ViaHM

Just tried that. Doesn't work. What could it be? I've even reinstalled XP and nothing. Is the card defective?
