Why do spammers put your e-mail username in the title?

I can maybe see trying to make a more personalized e-mail by putting one's first name in a title..though I've never gotten an e-mail from a friend who put it in a title..but when you put something like "Syringer, want cheap medicine?" it's easily discounted as spam.
