There are a few things that could induce lag in the audio. The first thing I would do however would be unplug the cable box for a few minutes and let it reboot.
Are you splitting the cables cable?
Does your receiver have a lip sync / time delay adjustment?

It could be the cable company DVR. There are quite a few models out that have poor firmware and can lose lip sync.
