Who here has experience with copyright registrations??? Anyone?

I'll try to keep this brief:
If you're in a band or an indie musician and have dealt with submitting a Form SR or PA and CDs for copyright then I've got a few questions that I hope you can answer for me...

Regarding form SR:
1) Areas 1-7 seem pretty straight forward, but if you list multiple authors in Section 2, who fills out and signs Section 8? (The first authtor, any author, all aurthors? Does it matter who?)

2) For a compilation (including many contents titles). How does the CD/Jewel case label need to read. Is permenant marker on the face of a CDR good enough?

3) Although I don't need to, can I / should I package printed lyrics with the CDs, and if I do does that require a separate registration form?

Well, I think I may have found the answer to #1

Quoted from Circular 55:
Space 8 (Forms PA and SR) or Space 10 (Form TX):
The application must bear the original signature of a person who is authroized to file the claim and should be dated.
