I have a standard 2 pin HD monitor cable on my case and I want to use it with my Ultra66 controller card. Which of the 4 pins do I use to do this and which way do I orient the plug red/black wires? Thanks.

Not sure, but why won't you experiment? While the HDD is active switch around the wires until it works.

Page 2-1 of the FastTrack 66 manual gives a real crappy desciption of the 'connectors' and board layout.
The 4-pin header is at the front (opposite the backplate) of the card; 4 small gold pins bent at 90 degrees. The center two pins are Ground and the outer two pins are 'Power'.
If I remember correctly (from when I installed my card) the 2 pins closest to the PCI slot side of the card is for IDE 1.
