where can i get upc for Epson Stylus Photo 820?

I bought the Epson Stylus Photo 820 and I sent in the rebate to Coppell, Texas but Staples has lost my forms.

I can't find my copy of the UPC code but I'm trying to see if I can apply via staplesrebates online. Where can I get the UPC so I can try this?
