I would go with "First Name Last Name"
Ms - Married but Separated
Mrs - Married

It?s hard to know what someone will find offensive, so stick with first name last name....

Personally, I would telephone the department and find out. If however you are unable to find out, then you should assume "Ms". :)

If I'm not mistaken, while reading Dear Abbey years ago (when you are in the Army out in the field with nothing else to read but Stars and Stripes, you know what I mean) the same question came up. Abbey consulted an expert on etiquette and stated that the use "Ms." was best if the marital status was unknown.
