Ms - Married but Separated
Mrs - Married

It?s hard to know what someone will find offensive, so stick with first name last name....

Personally, I would telephone the department and find out. If however you are unable to find out, then you should assume "Ms".

If I'm not mistaken, while reading Dear Abbey years ago (when you are in the Army out in the field with nothing else to read but Stars and Stripes, you know what I mean) the same question came up. Abbey consulted an expert on etiquette and stated that the use "Ms." was best if the marital status was unknown.
