When someone you don't know very well says something highly insulting to you...

I would step away from someone who needs to be insulting or a jerk. I try to stay away from people who are unpleasant.

1) what did they say
2) is it true in a sense
3) why do u care if it isnt true
4) you obviosly already know the answer to this question
5) you have to take into account their tone of voice, as well as any visual expressions they may have given at the time

Yes, or they're so trashy that they actually think that's appropriate behavior. In either case, they're usually not worth getting to know better.

Depends on the comment, and the tone. Its possible the person thinks you two have a better relationship that you think you do....if that makes sense.

People get shot for doing that sort of stuff, I however either insult back, or refrain from talking to said person.

It has taken years of trial and error, but I think of all the times I've stuck my foot in my mouth, spoke out of turn, etc. Then I temper my reply, or simply leave it alone.

Easy way to handle it. Find something wrong with them, lay down a monster insult, then say, "I'm just kidding too."
