What personal info do you put on there? Name, address, email address? Consider me moderately paranoid, just put your email address at the top of the resume?

Recruiters like to be able to give a phone call, in my experience, so it's an advantage to have a phone number up there. If you choose to just use an email address (and I always put one I don't mind getting junk mail in) isn't terrible.

I pulled my phone number from both the doc itself, and put a fake one in the resume form. I got tired of 3-5 Helpdesk offer phone calls per day.

So name and email address would be acceptable. I'm a little leary of putting my name on there, I know my current employer uses Monster. And I'd rather not deal with 20 questions from them just because I'm testing the water.

You don't even need your name on there. It's perfectly acceptable to have an anonymous resume with just email address, and it can be a masked email address through Monster, like craigslist uses.

yes i believe monster allows anonymous resume and then interested parties contact you through monster. you can then contact the party if you want.
