When I leave the bathroom.

Before handling food that I'm cooking, and while I'm cooking and have handled raw meat/fish/poultry (or utensils/cutting boards/etc that were used with raw meats) and will now be handling something else to prevent cross-contamination.

Before I go to sleep.

I am anal when it comes to washing with soap because I know and can feel the soap stay on my hands if I don't rinse long enough. Yes, I'm OCD about it.

After the bathroom. A quick wash if I went for a #1, and a more thorough wash for #2, especially if I got some on my hands. Same with pee, if I get some on my hands somehow then I do a more thorough wash.

I also wash after working outside, or after wearing gloves for a long time. (hands feel all icky and sweaty otherwise).

When I worked at the hospital I washed my hands any time I stepped out of the office if I touched anything from an elevator button to a railing, or a computer even. If there's one place to catch weird stuff, it's at a hospital.
