when applying thermal goop, should i remove the 4 little pads??

leave em, it doesn't hurt any bit. It only helps you level your heatsink

No, those are there to help prevent the heatsink from moving around and damaging the core. You should only apply the thermal grease on the core.

Leave them on unless you do this: make something better than the pads
